N2 - To the Editor: The article by Odio et al. on dexamethasone in children with bacterial meningitis (May 30 issue)* suggests, if I read it correctly, that 16 patients in the group presented in coma and that at least some of them had papilledema, pupil asymmetry, and decorticate or decerebrate posturing. If the protocol was adhered to, many of these patients would have had one, two, or possibly three lumbar punctures within the first 24 to 48 hours. If this is so, may I be permitted to question the ethics of such a protocol, given the known hazards of lumbar puncture.
