N2 - PURPOSE OF REVIEW: The purpose of this literature review is to assess the data and conclusions of recent investigations in disparity of care as relates to total joint arthroplasty. RECENT FINDINGS: Numerous investigations have confirmed the existence of significant disparities in the rates of total hip arthroplasty among different age, racial, ethnic, socioeconomic, and geographic populations. These disparities may be attributed to various causes including differing prevalence of arthritis, economic constraints, medical co-morbidities and contraindications to surgery, deficient knowledge and expectations regarding surgery, cultural differences, and lower referral rates from primary-care providers to orthopaedic surgeons. SUMMARY: Any potential solutions to such disparities will require both patient-level education as well as system-based changes in the delivery of arthroplasty care. All patients with severe hip arthritis deserve full and equal access to this highly effective surgical procedure.
