Abstract

Rambo (1958) presented a technique for primary closure of the radical mastoidectomy wound to eliminate postoperative maintenance. The method consisted of filling the radical mastoidectomy cavity and the endaural opening with a pedicled temporalis muscle flap. Laterally based anterior and posterior canal skin flaps were then laid across the muscle flap without primary suturing. Rambo's procedure and subsequent techniques have utilized either closure over a temporalis muscle flap, abdominal fat graft, or across an open air cavity. A modification of Rambo's technique of primary closure is offered as a method of obtaining a secure watertight seal of the external auditory canal that is both reliable and aesthetic with applications in neurotologic and skull base cases as well as in cases of chronic otitis media.
