The effect of age, gender and loading conditions on mitral annulus motion assessed by Doppler tissue imaging in normal subejects

Abstract

Mitral annulus motion (MAM) has been shown to mirror the mitral inflow pattern and has been lately used for assessing left ventricular (LV) filling dynamics. No systematic studies have evaluated the influence of age, gender and loading conditions on MAM. We sought to investigate the influence of these variables on the pattern of MAM after controlling the confounding effects of BP and HR. Eighty-three normal subjects, 39 men and 44 women, age 17-87, all in sinus rhythm, underwent Doppler echocardiography and Doppler Tissue Imaging (DTI) interrogation of the lateral and medial MAM recorded from 4 chamber apical view. MAM shows a positive systolic apically directed wave(s) and two negative waves atrially directed (e' and a') corresponding to E and A waves of the mitral inflow. Peak velocities (PkV) and time velocity integrals (TVI) of every MAM wave were measured. A tilt table was used for preload manipulation to examine its effect on MAM. A paired t-test was used to assess the effect of preload on MAM and multiple linear regression to evaluate the influence of age and gender on MAM. Results: 1. Age was significantly negatively associated with e'/a' (r=.25, p=.002), PkVe' (r=-.58, p=.0001), and TVIe' (r=-.42, p=.003). 2. Women had significantly lower values of PkVs)p=.026) and TVIe' (p=.013). 3. In orthostatism, the variables with significantly lower values than in supine position were: TVIs (p=.01), PkVe' (p=.008), TVIe' (p=.001), and PkVa' (p=.014). Conclusion: MAM is age, gender and preload dependent. These parameters should be considered in the evaluation of MAM by DTI.
