Abstract

To the Editor: We wish to highlight a shortcoming of the study by Packer et al. (July 1 issue)¹ regarding the withdrawal of digoxin from patients with chronic heart failure treated with angiotensin-converting-enzyme (ACE) inhibitors. The authors state that during the single-blind run-in period the doses of ACE inhibitor were increased “so that a daily dose of at least 25 mg of captopril or 5 mg of enalapril was achieved.” This dosage level was to provide “optimal clinical benefits.” During the study, patients received an average daily dose of 73 mg of captopril or 13 mg of enalapril in the…
